What to bring or send

Unless your application is already on file, NPHE needs proof of income for everyone in the household (recent pay stubs, benefit award letters, or tax return) before your fee is set. Adults 18 and over also need to complete the drug & alcohol awareness class, or have an approved exemption, before or within 30 days of the first service.
